Twin Cities (RSC) Relocation Service Coordination

The Relocation Service Coordination (RSC) Program is funded through Minnesota Medical Assistance (Medicaid), with little to no costs to our clients. When people are living in an institution such as a nursing home, but are ready to move back into the community, Right At Home Service helps. We provide experienced case management that guides and supports people as they plan to move out of certain institutions and back into homes. To be eligible Medical Assistance beneficiaries should be moving out of a nursing facility, regional treatment center, hospital or ICF/DD (intermediate care facility for persons with developmental disabilities).

Services vary according to individualized service plans.

  • Communicate with you and other necessary people to carry out the plan
  • Coordinate with the institution discharge planner
  • Develop and review a relocation plan with you
  • Make referrals to needed services and supports
  • Travel and complete documentation that is necessary to develop and carry out the plan
